 To request an appointment fill out the form below or contact us by text, email or phone with your name, an idea of your location or address, and a detailed message about your pet including age, estimated weight and a brief medical history. We understand sometimes the need to postpone, we ask that you please contact us 24 hours in advance if possible for cancellation. Please always leave us a voicemail or send us a text message, 360-746-9635 and we will confirm cancellation.

*Every animal is unique. Upon arrival Dr. Graffe reserves the right to perform a physical exam and/or exam consultation on your pet at your home and may refuse to euthanize based on his medical assessment.
